1. Who we are and what this policy covers
Patiendra Home Nurses arranges scheduled home nursing care across Northern Uganda, subject to staff availability and travel arrangements. This policy explains how we handle information about website visitors, people requesting care, patients, authorised representatives and people paying for care.
Our operating base is Layibi Central, Tegwana Pece Division, Gulu City, Northern Uganda. Contact management using the details below for privacy questions. Staff confidentiality responsibilities are addressed separately in our General Non-Disclosure and Privacy Undertaking.
2. Information we collect
When you request care, we ask for your name and phone number, the patient's name and age or age group, the service requested, a brief description of the help needed, the care location, preferred date and time, and privacy consent. Optional fields include another phone number, directions, a map link and alternative scheduling preferences. A client account is not required.
Required fields are identified on the form. Without enough contact, care and location information, we may be unable to review the request. Please do not send full medical records, identity documents, passwords, mobile-money PINs or unnecessary sensitive details through the initial form. Do not put patient information in a website address.
If care proceeds, records may include assessments, agreed care plans, quotations, approvals, payment references and verification, nurse assignments, schedules, visit notes, incidents, complaints and balances. These records may contain health and financial information.
3. Where information comes from
Information may be supplied by you, the patient, an authorised representative or the nurses involved in care. Where appropriate and lawful, relevant information may also come from a treating professional or from payment records used to verify a transaction. We seek information relevant to the particular request or service.
4. How and why information is used
We use relevant information to respond to enquiries, review care needs, arrange assessment, prepare quotations, record approval, verify payments, arrange nurses and visits, maintain care records, handle concerns and meet applicable legal, accounting and professional obligations. Technical information helps operate and protect the website.
Consent is obtained where required. Some processing may be necessary for care arrangements or permitted or required by law. Health information is subject to additional confidentiality requirements. Website consent does not replace the consent or other lawful authority needed for clinical care.
Care details are not collected for advertising. Confidential information must not be used for unauthorised personal gain, publicity or disclosure.
5. Requesting care for another person
Provide information about another person only when you have authority or another lawful basis to do so. Explain your relationship to the patient when the team contacts you. A relative, contact person or payer does not automatically have authority to make all decisions or receive all patient information.
For children or people unable to give the relevant consent, the appropriate parent, guardian or other lawful authority must be established before proceeding where required. The team may seek further confirmation; submitting the form does not establish that authority on its own.
7. Photographs, recordings and messaging
Our confidentiality rules prohibit taking, retaining, forwarding or publishing patient photographs, recordings, home details or care records without the necessary lawful authorisation. A care request does not authorise publicity, testimonials or social-media use of patient information.
Phone, email, WhatsApp and map links may open another application or service, whose own privacy terms apply. Use those channels carefully and share only what is needed. Please contact the team to agree an appropriate way to provide detailed records. Do not send payment PINs or account passwords.
9. Security and storage
The website uses HTTPS, protected administrator sessions and access-controlled administration pages. Staff are required to take reasonable care with records, devices and messages, and to report lost documents, misdirected messages or suspected unauthorised access. No website or communication method can guarantee absolute security.
Records are held in the systems used to operate the service and may also exist in authorised care documentation and communications. Where a service involves handling data outside Uganda, the applicable legal requirements for that processing must be met. Contact management for information about storage and service-provider arrangements relevant to your records.
10. How long records are kept
Records should be kept only for as long as needed for their lawful purpose. Relevant considerations include ongoing care, follow-up, payment reconciliation, professional recordkeeping obligations, complaints or legal claims, and any requirement to preserve evidence. Different types of records can require different periods.
This notice does not promise a single retention period or automatic deletion date. Contact management for the retention basis that applies to your records or to request deletion. A request may be limited where records must lawfully be retained; any applicable reason will be explained. Staff must return records and cease unauthorised access when their engagement ends, with deletion of unauthorised personal copies subject to lawful retention requirements.
11. Your choices and privacy rights
Subject to applicable law, you may ask whether we hold your information, request access, ask for inaccurate information to be corrected, or request deletion or restriction where appropriate. You may also raise an objection to a particular use. Where processing relies on consent, you may withdraw that consent; this does not undo earlier lawful processing or remove a separate legal obligation to keep records.
Contact management and describe your request. We may need proportionate proof of identity or authority to protect the patient and other people. We will explain any lawful limits and any effect on the care we can arrange. Please do not send identity documents unless requested through an appropriate channel.
12. Questions, concerns and complaints
Email patiendrahomenurses@gmail.com or call 0785 855 559 and ask for the person responsible for privacy. Include a request reference if available and a safe way to contact you, rather than detailed health information in an initial message.
You may also raise a privacy complaint with Uganda's Personal Data Protection Office. Contacting us does not prevent you from approaching the regulator or exercising other legal rights.
13. Updates to this policy
The date above identifies this version. We will update this page when relevant practices or requirements change. A revised notice does not retrospectively authorise a new use of information that requires separate consent or another lawful basis. See our Terms of Service for care-request and service arrangements.
